Hello fellow teachers,

Does anyone use an "evaluation form" when a student quits? What kinds of questions do you ask the parent/child to write about you as a teacher?
Do you ask for ratings on differnt things, multiple choice, etc?
I would like to begin to do this to help me grow as a teacher. Where do I start?

Very brave of you!! I think the evaluation may depend on why a student is quitting and may not reflect upon you as a teacher. In other words, say the student is quitting because they never practice (nothing to do with you) or the parents got mad because you charged them a late fee for a late payment (just an example!!). You then give them an evaluation form and you may get very different answers as opposed to students who are quitting because they are leaving for college!

I think if it were me, I would give them the evaluation form before they quit, while you are still teaching them.

I few months ago I read a really good exit form on another forum, but I'm not sure where. Does anyone else remember seeing it? It was one of the best I've seen, but I forgot to save it.

She suggests using them for year-end for all students. We haven't done this, but are considering using them. Have you had success with these? Has the feedback you received made a difference in how you run your studio or does it just open up a "can of worms"?
